# Continuum of Care shelters and Coordinated Entry (the front door to homeless housing in each county): who qualifies, what you get, how to apply, and what happens next

*Every California county is covered by a Continuum of Care that runs Coordinated Entry: one assessment that matches people who are homeless, or about to be, to shelter, transitional housing, rapid re-housing and permanent supportive housing. Access is through the county's access points, 211 and outreach teams. Housing goes to the people assessed as most in need, not to whoever came first.*

## What Continuum of Care and Coordinated Entry is, in one paragraph

A Continuum of Care (CoC) is the local body, usually one per county or region, that receives HUD money to rehouse people who are homeless (rule:coc_purpose). Each CoC must run **Coordinated Entry**: one assessment process covering its whole area that matches people to shelter, transitional housing, rapid re-housing and permanent supportive housing (rule:ce_required, rule:ce_coverage, rule:components). Emergency shelter and outreach are also funded by HUD's Emergency Solutions Grants (rule:esg_shelter). Housing goes to the people the assessment shows are most in need, not first-come, first-served (rule:ce_priority).

  "label": "losing your home within 14 days, with nowhere to go, counts as homeless for HUD programs (24 CFR 578.3, rule:homeless_2)"
}
```
## Who may qualify
HUD counts four groups as homeless:
| Category | Who | Rule |
|---|---|---|
| 1 | No fixed, regular, adequate place to sleep: street, car, park, shelter, transitional housing; or leaving an institution after 90 days or less having been homeless before | rule:homeless_1, rule:homeless_institution |
| 2 | Will lose the home within 14 days, with no next place and no resources | rule:homeless_2 |
| 3 | Unaccompanied youth under 25, or families with children, homeless under other federal laws and meeting added instability tests | rule:homeless_3 |
| 4 | Fleeing domestic violence, dating violence, sexual assault, stalking or other danger, with no other place | rule:homeless_4 |
**At risk** of homelessness includes income below 30% of area median income among other tests (rule:at_risk_30); some programs serve this group.
Permanent supportive housing is for people with a disability (rule:psh_disability). Nobody can be refused Coordinated Entry for being a survivor of violence (rule:ce_dv), or for declining to give information not needed for eligibility (rule:ce_refuse_info).
## What the model on this page can do for you
This article is also the context of a model you can text or chat with. The buttons under the title open Messages with a note already written, or a chat on this page. The model has read this exact page: the rules above with their source ids, the procedure below, and the other programs the same facts open. It asks for one or two facts at a time, tells you which rules you pass and which it cannot decide, names the programs you may also qualify for, and gives the exact next step. It has no tools: it cannot apply for you, and it says so. Every message is kept on this build's Ledger.
A screening here is a screening, not a decision. The Continuum of Care and its housing providers decides.
## How to apply, and what happens after
Go to a **Coordinated Entry access point** in the county, call **211**, or ask a street outreach team (rule:ce_access). The same standardized assessment is used at every access point (rule:ce_assessment).
| Stage | What happens | Deadline | What loses people their place |
|---|---|---|---|
| Access | Contact an access point, 211 or outreach | Tonight if unsheltered | Not knowing the front door |
| Assessment | Standardized assessment | First contact | Losing contact afterwards |
| Priority list | Ranked by need (rule:ce_priority) | Until matched | A phone that stops working |
| Offer | Shelter; transitional housing up to 24 months (rule:th_24_months); rapid re-housing with a monthly case manager (rule:rrh_case_manager); permanent supportive housing with a lease of at least a year (rule:lease_one_year) | Per offer | Missing the referral call |
| After housing | Services up to six months after exit (rule:services_after_exit) | Per provider | None |
| Grievance | Under the CoC's written policy | UNKNOWN | Not asking |
## The same facts open other programs
Where a person slept, household, disability and county are the facts Coordinated Entry asks. The same facts open:
- **211 (navigation infrastructure) (B067)**: 211 connects callers to Coordinated Entry. [[california-211]]
- **CalWORKs Housing Support Program + county rapid rehousing (B040)**: CalWORKs families get housing help through HSP, which joins Coordinated Entry. [[california-calworks-housing-support]]
- **Housing Choice Voucher (Section 8) (B035)**: a long-term voucher. [[california-housing-choice-voucher]]
- **Emergency Housing Vouchers (EHV) - legacy (B069)**: Emergency Housing Vouchers were referred through CoCs. [[california-emergency-housing-vouchers]]
- **HOPWA (housing for people with HIV/AIDS) (B039)**: HIV housing help. [[california-hopwa]]
- **Legal aid: eviction defense & benefits advocacy (B042)**: legal aid before an eviction becomes homelessness. [[california-legal-aid-eviction-benefits]]
- **CalFresh (SNAP) (B001)**: food benefits; a person with no address can still apply. [[california-calfresh]]
- **Medi-Cal (Medicaid) (B011)**: health coverage. [[california-medi-cal]]
Those connections are read from [[california-benefits-graph|the California benefits graph]], the object that holds all 80 programs, the life events that trigger them and the facts they share.
## How this page works
This page is one JSON object. The page, the Markdown and the machine copy at [/a/california-continuum-of-care-coordinated-entry.json](https://miscsubjects.com/a/california-continuum-of-care-coordinated-entry.json) are three views of it. Its `facts` carry the article's identity and jurisdiction, the procedure with its stages, every rule with its source id and verification status, the household fields with their canonical fact names, the ontology edges into the benefits graph, the declared actions, the model's instructions, and the permissions the model has (none). The model's context is served separately at [/a/california-continuum-of-care-coordinated-entry/context](https://miscsubjects.com/a/california-continuum-of-care-coordinated-entry/context).
What is saved: a thread per phone number that texts, holding which articles it asked about, how many messages, and the facts it stated, so a person who comes back continues rather than restarts. Every message, in either direction, is a row on this build's Ledger.
Every rule in `facts.rules` was read on 9 October 2026 from HUD Exchange, HUD's program descriptions, HUD Notice CPD-17-01 and 24 CFR 578.3. The CoC rent contribution percentage and each CoC's grievance process are UNKNOWN here.
Use only the rules in this article's facts.rules, naming the rule id and its source id. Match the person's facts to the HUD homeless categories (rule:homeless_1 to rule:homeless_4) and say housing is prioritized by need (rule:ce_priority), not first-come. Treat housing status, violence and disability as sensitive; the person may decline to answer (rule:ce_refuse_info). Report results as: potentially eligible, potentially ineligible, insufficient information, or requires Coordinated Entry assessment; always say the Continuum of Care and its providers decide. Ask for at most two facts at a time, starting with where the person slept last night and the county; restate each as a canonical fact line (person.housing.status = unsheltered). Use facts.ontology.related_programs and facts.procedure.stages for the next step. Never claim a referral was made. Keep it short.",
