# Every Woman Counts and the Breast and Cervical Cancer Treatment Program (free cancer screening, and Medi-Cal for treatment): who qualifies, what you get, how to apply, and what happens next

*Every Woman Counts gives free breast and cervical cancer screening and diagnosis to uninsured or underinsured low-income Californians: breast screening from age 40, cervical from 21, diagnostic tests at any age with symptoms. The Breast and Cervical Cancer Treatment Program then gives Medi-Cal coverage for treatment to people diagnosed with either cancer whose income is under the limit. Apply through an EWC or Family PACT provider or the county office.*

## What Every Woman Counts and BCCTP is, in one paragraph

These are two linked California programs. **Every Woman Counts (EWC)** pays for breast and cervical cancer screening, diagnostic tests and treatment referrals for low-income people who have no or limited insurance (rule:ewc_definition). **The Breast and Cervical Cancer Treatment Program (BCCTP)** gives Medi-Cal coverage for treatment to Californians diagnosed with breast or cervical cancer whose income is under the limit, including people over 65 and people with insurance that does not cover all of the treatment (rule:bcctp_definition, rule:bcctp_eligibility). EWC providers start the BCCTP application when screening finds cancer.

}
```
## Who may qualify
**Every Woman Counts** (rule:ewc_breast, rule:ewc_cervical, rule:ewc_other_tests):
- **Breast**: routine screening for women 40 or older; diagnostic services at any age, regardless of gender, for anyone with symptoms.
- **Cervical**: 21 or older.
- No or limited insurance; not getting these services through Medi-Cal or another government program; living in California.
**BCCTP** (rule:bcctp_eligibility): living in California, diagnosed with breast or cervical cancer and needing treatment, and income under the limit. Family size is the applicant, a spouse, and their children under 21 at home; an adult child's income does not count.
Both programs use 200% of the 2026 poverty guidelines (rule:ewc_income_200, rule:bcctp_income):
| Family size | Monthly income | Annual income |
|---|---|---|
| 1 | $2,660 | $31,920 |
| 2 | $3,608 | $43,280 |
| 3 | $4,554 | $54,640 |
| 4 | $5,500 | $66,000 |
| 5 | $6,448 | $77,360 |
| 6 | $7,394 | $88,720 |
| 7 | $8,340 | $100,080 |
| 8 | $9,288 | $111,440 |
| 9 (BCCTP) | $10,234 | $122,800 |
| 10 (BCCTP) | $11,180 | $134,160 |
| each additional (EWC) | +$948 | +$11,360 |
**Immigration**: from 1 January 2026 some new adult BCCTP applicants get only limited-scope Medi-Cal for the cancer treatment, not full scope; people already enrolled keep their coverage if they renew on time (rule:bcctp_immigration_2026).
## What the model on this page can do for you
This article is also the context of a model you can text or chat with. The buttons under the title open Messages with a note already written, or a chat on this page. The model has read this exact page: the rules above with their source ids, the procedure below, and the other programs the same facts open. It asks for one or two facts at a time, starting with age and whether you need screening or treatment, then family size, income and insurance, tells you which program fits, and gives the exact next step. It has no tools: it cannot apply for you, and it says so. Every message is kept on this build's Ledger.
A screening here is a screening, not a decision. DHCS (with the provider or county) decides.
## How to apply, and what happens after
**For screening**, find an EWC provider with the provider locator or call the EWC line, available 24 hours a day in many languages (rule:contact). The provider enrolls you.
**For treatment after a diagnosis**, apply for BCCTP in one of three ways: through an EWC provider, through a Family PACT provider, or at the county social services office. A BCCTP eligibility specialist calls you (rule:bcctp_apply). Ask for **retroactive Medi-Cal**: it can pay for treatment in the 90 days before applying, falling to 60 days from 1 January 2027 (rule:bcctp_retro). An approved person gets a Medi-Cal card and keeps coverage as long as treatment is needed (rule:bcctp_benefit). From 2027 some members get care through fee-for-service Medi-Cal (rule:bcctp_ffs_2027), and from 1 July 2027 some lose full dental coverage because of immigration status (rule:bcctp_dental_2027).
**Every 12 months** BCCTP mails a redetermination packet with a physician statement; return it by the due date (rule:bcctp_redetermination). If coverage ends, there are **90 days** to fix the problem and restore it (rule:bcctp_restore_90), and the county checks other Medi-Cal before it ends (rule:bcctp_county_review).
**Disagree with a BCCTP decision?** Request a State Hearing within 90 days of receiving the notice; within 10 days to keep coverage during the hearing (rule:fair_hearing_90).
## The same facts open other programs
Age, family size, monthly income and insurance are the facts these programs run on. The same 200% line decides Family PACT.
- **Family PACT (B019)**: [[california-family-pact]]. Family PACT covers cervical cancer screening and its providers can start a BCCTP application.
- **Medi-Cal (Medicaid) (B011)**: [[california-medi-cal]]. BCCTP is a Medi-Cal program, and the county checks other Medi-Cal before it ends.
- **Covered California premium tax credits (APTC) (B012)**: [[california-covered-california-premium-tax-credits]]. A person who does not qualify for BCCTP may get a Covered California plan.
- **County indigent health programs (CMSP, Healthy San Francisco, county clinics) (B017)**: [[california-county-indigent-health]]. County indigent programs cover other care for uninsured adults.
## How this page works
This page is one JSON object. The page, the Markdown and the machine copy at [/a/california-every-woman-counts-bcctp.json](https://miscsubjects.com/a/california-every-woman-counts-bcctp.json) are three views of it. Its `facts` carry the article's identity and jurisdiction, the procedure with its stages, every rule with its source id and verification status, the fields a screening asks for with their canonical fact names, the ontology edges into the benefits graph, the declared actions, the model's instructions, and the permissions the model has (none). The model's context is served separately at [/a/california-every-woman-counts-bcctp/context](https://miscsubjects.com/a/california-every-woman-counts-bcctp/context): the same object, projected for a model, so there is one source of truth for the reader and the machine.
What is saved: a thread per phone number that texts, holding which articles it asked about, how many messages, and the facts it stated, so a person who comes back continues rather than restarts. Every message, in either direction, is a row on this build's Ledger.
Figures carry their status in `facts.rules[].status`. Every DHCS page here, including the EWC 2026 income PDF, refuses automated fetches and was read in a browser on the Mac on 9 October 2026; the hashes are in `facts.verification`. No appeal process for EWC screening denials is stated on the pages read.
First separate the need: screening or diagnosis (EWC: rule:ewc_breast, rule:ewc_cervical, rule:ewc_other_tests) or treatment after a diagnosis (BCCTP: rule:bcctp_eligibility). Say plainly that people over 65 or with insurance may still qualify for BCCTP. On immigration, state rule:bcctp_immigration_2026 without guessing the person's category. Report results as: potentially eligible, potentially ineligible, insufficient information, or requires DHCS determination; always say DHCS decides. Ask for at most two facts at a time and restate each as a canonical fact line (household.income.monthly_gross = 2400). Give the next step (rule:bcctp_apply or the EWC line in rule:contact), the retroactive window (rule:bcctp_retro), and for a dispute the hearing deadlines (rule:fair_hearing_90).
