# In-Home Supportive Services (IHSS) (paid in-home care for aged, blind and disabled Californians on Medi-Cal): who qualifies, what you get, how to apply, and what happens next

*IHSS pays a provider, who can be a family member, to help aged, blind and disabled people on Medi-Cal with daily tasks at home, up to 283 hours a month for the most impaired. The county assesses need at a home visit. Apply with form SOC 295 at the county IHSS office.*

## What IHSS is, in one paragraph

In-Home Supportive Services (IHSS) pays for help at home for people who are aged, blind or disabled, so they can stay home instead of moving to a care facility (rule:who). The county assesses which tasks a person needs help with, such as bathing, dressing, meals, cleaning, shopping and getting to medical appointments, and authorizes a number of hours each month. The person hires the provider, who can be a relative, and the state pays the provider. It is a Medi-Cal program run by each county.

```widget
{
  "type": "stat",
  "value": "283 hours",
  "label": "the most IHSS hours a severely impaired person can be authorized each month; 195 if not severely impaired (CDSS regulation, rule:max_hours)"
}
```

## Who may qualify

A person may qualify who meets all of:

- lives in California (rule:resident);
- has a Medi-Cal eligibility determination (rule:medi_cal);
- lives at home or a place of their own choosing, not a hospital or care facility (rule:own_home);
- is aged, blind or disabled and needs help to stay safely at home (rule:who);
- returns a Health Care Certification (SOC 873) from a licensed health care professional (rule:health_care_certification).

**Hours.** The county social worker sets the hours at a home visit (rule:home_assessment). Monthly limits through an individual provider:

| Recipient | Maximum hours per month |
|---|---|
| Severely impaired | 283 |
| Not severely impaired | 195 |

(rule:max_hours, rule:absolute_max). **Pay.** The provider's hourly wage is set county by county (rule:provider_wage).

## What the model on this page can do for you

This article is also the context of a model you can text or chat with. The buttons under the title open Messages with a note already written, or a chat on this page. The model has read this exact page: the rules above with their source ids, the procedure below, and the other programs the same facts open. It asks for one or two facts at a time, starting with Medi-Cal status and the tasks that need help, tells you which rules you pass and which it cannot decide, helps list each task and the time it takes before the home visit, and gives the exact next step. It has no tools: it cannot apply for you, and it says so. Every message is kept on this build's Ledger.

A screening here is a screening, not a decision. The county decides.

## How to apply, and what happens after

Submit an application (SOC 295) to your county IHSS office (rule:apply_where). The doctor's Health Care Certification must reach the county before any hours are authorized (rule:health_care_certification).

| Stage | What happens | Deadline | What loses people their hours |
|---|---|---|---|
| Screen | Residence, Medi-Cal, own home, daily needs | Minutes | No Medi-Cal determination yet |
| Apply | SOC 295 to the county IHSS office | Same day | Waiting for a Medi-Cal decision before applying |
| Certification | SOC 873 from a health care professional | Before authorization | Form never returned |
| Home visit | Social worker assesses each task | UNKNOWN | Underreporting needs |
| Decision | Notice with services and monthly hours (rule:hours_notice) | UNKNOWN | Accepting hours below need |
| Hire | Recipient hires and supervises the provider (rule:hire_provider) | Before work starts | Provider not enrolled |
| Reassess | Every year, or on request (rule:annual_reassessment, rule:reassess_on_request) | Yearly | Hours cut at reassessment |
| Appeal | State hearing | 90 days from the notice (rule:hearing_90) | Missing the window |

## How this page works

This page is one JSON object. The page, the Markdown and the machine copy at [/a/california-ihss.json](https://miscsubjects.com/a/california-ihss.json) are three views of it. Its `facts` carry the article's identity and jurisdiction, the procedure with its stages, every rule with its source id and verification status, the fields with their canonical fact names, the ontology edges into the benefits graph, the declared actions, the model's instructions, and the permissions the model has (none). The model's context is served separately at [/a/california-ihss/context](https://miscsubjects.com/a/california-ihss/context): the same object, projected for a model.

What is saved: a thread per phone number that texts, holding which articles it asked about, how many messages, and the facts it stated, so a person who comes back continues rather than restarts. Every message, in either direction, is a row on this build's Ledger.

Figures carry their status in `facts.rules[].status`. The 283- and 195-hour limits are from the CDSS regulation page (MPP Division 30), read on 9 October 2026; county wage rates are UNKNOWN here.

Ask for at most two facts at a time, starting with Medi-Cal status and the daily tasks that need help; restate each as a canonical fact line (person.address.residential.state = CA). Help the person list each task and the time it takes before the county home visit; never estimate authorized hours beyond the 195 and 283 limits in rule:max_hours.
