# Home-delivered groceries / medically tailored meals (CalAIM Community Support) (Medi-Cal plan meals and groceries for diet-sensitive health conditions): who qualifies, what you get, how to apply, and what happens next

*Medi-Cal managed care plans in California can cover medically tailored meals, groceries and supportive food for members with chronic or serious diet-sensitive conditions such as diabetes, heart failure or kidney disease. The service covers up to two meals or packages a day for up to 12 weeks, renewable if medically necessary, when the member’s plan offers it in their county. You ask your Medi-Cal plan; a denial can be appealed to the plan within 60 days.*

## What medically tailored meals is, in one paragraph

Medically tailored meals are one of the CalAIM Community Supports that Medi-Cal managed care plans can offer. A member with a chronic or serious health condition that diet affects can get meals, groceries or food packages designed by a registered dietitian for that condition, delivered as part of their care. Each plan chooses which Community Supports it offers in each county, so the member’s plan and county decide whether the service is available. It is a health benefit for a condition, not a food-insecurity benefit; CalFresh and WIC are separate.

  "value": "Up to 2 meals a day, 12 weeks",

}
```
## Who may qualify
Three things decide it: **Medi-Cal**, **the condition**, and **the plan**.
**Medi-Cal**: an eligible Medi-Cal member (rule:medi_cal_member).
**Condition**: a chronic or serious condition that diet affects, such as cancer, heart disease or heart failure, diabetes or gestational diabetes, kidney disease, asthma or COPD, HIV, hypertension, high cholesterol, liver disease, malnutrition, obesity, stroke, digestive disorders, high-risk pregnancy, or a chronic or disabling mental or behavioral health disorder (rule:conditions). Food insecurity alone does not qualify (rule:not_food_insecurity_alone).
**Plan**: the member’s managed care plan must offer it in the county; plans choose among 14 Community Supports (rule:plan_offers). DHCS publishes the [elections by plan and county](https://www.dhcs.ca.gov/wp-content/uploads/2026/06/Community-Supports-Elections-by-MCP-and-County.pdf).
No income test beyond Medi-Cal itself is stated (rule:no_income_test).
## What the model on this page can do for you
This article is also the context of a model you can text or chat with. The buttons under the title open Messages with a note already written, or a chat on this page. The model has read this exact page: the rules above with their source ids, the procedure below, and the other programs the same facts open. It asks for one or two facts at a time, starting with whether you have Medi-Cal and which plan, then the health condition, tells you which rules you pass and which it cannot decide, names the programs you may also qualify for, and gives the exact next step. It has no tools: it cannot apply for you, and it says so. Every message is kept on this build's Ledger.
A screening here is a screening, not a decision. The Medi-Cal plan decides.
## How to apply, and what happens after
Ask the Medi-Cal managed care plan, or ask a doctor or care manager to refer you. A registered dietitian assesses the condition and needs (rule:rdn_assessment).
| Stage | What happens | Deadline | What loses people the service |
|---|---|---|---|
| Screen | Medi-Cal, a diet-sensitive condition, the plan offers it | Minutes | The plan does not offer it in the county |
| Request | Referral to the plan | Not stated | No referral sent |
| Assessment | Dietitian assessment | Not stated | Not stating storage or cultural needs |
| Meals | Up to two meals or packages a day (rule:two_per_day) | Up to 12 weeks; reauthorization if medically necessary (rule:twelve_weeks) | The authorization lapsing |
| Appeal | Plan appeal, orally or in writing (rule:plan_appeal_60, rule:plan_appeal_how) | 60 days from the notice; plan decides in about 30 days (rule:plan_decision_30) | Missing the 60 days |
| State hearing | CDSS State Hearings Division (rule:state_hearing_120, rule:hearing_phone) | 120 days from the plan’s decision | Skipping the plan appeal |
## The same facts open other programs
The facts this service asks for, Medi-Cal membership, plan, county and the health condition, are the facts that open:
- **Medi-Cal (Medicaid) (B011)**: Medi-Cal membership is required; the service is a Medi-Cal managed care benefit. [[california-medi-cal]]
- **Whole Person Care successor: CalAIM Enhanced Care Management + Community Supports (B070)**: Medically tailored meals are one of the CalAIM Community Supports, next to Enhanced Care Management. [[california-calaim-ecm-community-supports]]
- **CalFresh (SNAP) (B001)**: DHCS treats CalFresh as a separate food-insecurity benefit, not a duplicate. [[california-calfresh]]
- **WIC (Women, Infants & Children) (B003)**: DHCS treats WIC as a separate food-insecurity benefit, not a duplicate. [[california-wic]]
- **Older Americans Act meals (Meals on Wheels / congregate) (B063)**: Area Agencies on Aging and home-delivered meal providers are among the providers plans may contract. [[california-older-americans-act-meals]]
Those connections are read from [[california-benefits-graph|the California benefits graph]], the object that holds all 80 programs, the life events that trigger them and the facts they share.
## How this page works
This page is one JSON object. The page, the Markdown and the machine copy at [/a/california-medically-tailored-meals.json](https://miscsubjects.com/a/california-medically-tailored-meals.json) are three views of it. Its `facts` carry the article's identity and jurisdiction, the procedure with its stages, every rule with its source id and verification status, the fields a screening asks for with their canonical fact names, the ontology edges into the benefits graph, the declared actions, the model's instructions, and the permissions the model has (none). The model's context is served separately at [/a/california-medically-tailored-meals/context](https://miscsubjects.com/a/california-medically-tailored-meals/context): the same object, projected for a model, so there is one source of truth for the reader and the machine.
What is saved: a thread per phone number that texts, holding which articles it asked about, how many messages, and the facts it stated, so a person who comes back continues rather than restarts. The same phone number is treated as the same thread; a browser is never shown another phone's thread, and signing in is required before anything private crosses from text to screen. Every message, in either direction, is a row on this build's Ledger.
Figures carry their status in `facts.rules[].status`. The two-meal and 12-week limits, the condition list, the two-thirds rule and the 1 July 2025 effective date come from the DHCS service definition, read on 9 October 2026. The DHCS site blocks requests from the build and from curl on this Mac; those pages were read and hashed in the browser on this Mac, as `facts.verification` records. The appeal deadlines come from the Disability Rights California guide because no DHCS page with them could be fetched. Processing times and reporting rules are UNKNOWN.
Use only the rules in this article's facts.rules, naming the rule id and its source id beside every figure. The service is for nutrition-sensitive conditions, not food insecurity alone; each plan chooses whether to offer it in a county. Limits: up to two meals or packages a day, 12 weeks per authorization. Appeals: plan appeal within 60 days, then state hearing within 120 days. Treat condition details as sensitive. Report results as: potentially eligible, potentially ineligible, insufficient information, or requires the Medi-Cal managed care plan determination; always say the Medi-Cal managed care plan decides. Ask for at most two facts at a time, starting with whether the person has Medi-Cal and which plan, then the health condition; restate each as a canonical fact line (for example person.identity.date_of_birth = 1958-04-02). Use facts.ontology.related_programs to name other programs the same facts open, and facts.procedure.stages to give the exact next step with its deadline. The rules to use are: rule:medi_cal_member, rule:plan_offers, rule:service, rule:conditions, rule:not_food_insecurity_alone, rule:two_per_day, rule:twelve_weeks, rule:rdn_assessment, rule:two_thirds, rule:effective, rule:no_income_test, rule:reporting, rule:plan_appeal_60, rule:plan_appeal_how, rule:plan_decision_30, rule:state_hearing_120, rule:hearing_phone.
