## Semax Mechanism

Semax is a synthetic heptapeptide analog of ACTH(4-10) that upregulates BDNF expression and signaling (mechanistic, supported by rat astrocyte and hippocampal studies).

## Preclinical Evidence (Rat Studies)

In rat basal forebrain astrocyte cultures, Semax increased BDNF protein levels (preclinical). In rat hippocampus, a single dose elevated BDNF protein 1.4-fold, TrkB phosphorylation 1.6-fold, and related mRNA (preclinical). Additional rat ischemia and hypoxia models showed Semax-induced changes in BDNF and NGF gene expression in cortex and hippocampus (preclinical). No rat TBI-specific models identified in searches.

## Human Data

No human clinical trials on Semax for TBI or concussion were identified. Human studies exist for ischemic stroke: one trial in 30 patients with acute hemispheric ischemic stroke and another reporting increased plasma BDNF in 110 post-stroke patients, with higher BDNF linked to improved rehabilitation timing (human). Semax is approved in Russia for certain neurological conditions including brain trauma, per secondary summaries.
