Family PACT (free family planning and reproductive health care in California): who qualifies, what you get, how to apply, and what happens next

Family PACT gives free birth control, family planning counseling, STI and HIV testing, cervical cancer screening and limited fertility services to California residents of any age who can get pregnant or cause a pregnancy, with family income at or below 200% of the poverty level and no other coverage they can use. Enrollment is confidential and happens at an enrolled clinic or online.

What Family PACT is, in one paragraph

Family PACT (Family Planning, Access, Care, and Treatment) is a California program, run by the Department of Health Care Services' Office of Family Planning since 1997, that pays for family planning and reproductive health care for low-income residents who have no other coverage for it (rule:definition). Care is free and confidential and is given at enrolled clinics and doctors' offices; prescriptions are filled at any pharmacy that takes Medi-Cal (rule:services, rule:confidential, rule:pharmacy_ca_only).

Who may qualify

Four tests (rule:residency, rule:medical_need, rule:income_200, rule:no_other_coverage):

  1. California resident when applying.
  2. A medical need for family planning: able to get pregnant or cause a pregnancy, at any age.
  3. Income at or below 200% of the poverty guideline, for the basic family unit: the person, a spouse, and minor children at home. A minor counts only their own income, not the parents' (rule:family_unit).
  4. No other coverage for family planning, with exceptions: coverage with no contraceptive method, a student with no contraceptive coverage, an annual deductible the person cannot meet that day, an unmet Medi-Cal share of cost, or fear that a parent, partner or spouse will find out.

Income limits from 1 April 2026 (rule:income_table_2026):

Family sizeMonthly incomeAnnual income
1$2,660$31,920
2$3,608$43,280

7$8,340$100,080
8$9,288$111,440
each additional+$948+$11,360

A person who becomes pregnant is not covered until after the birth; the clinic can check Presumptive Eligibility for Pregnant People (rule:pregnancy).

How to apply, and what happens after

Take the pre-application survey, then apply online in the Family PACT portal or at any enrolled provider (rule:apply). A client gets a Health Access Programs (HAP) identification number, good for one year; it can be renewed after 10 months (rule:one_year). Family PACT may also pay for family planning in the 3 months before enrolling, on the Retroactive Eligibility Certification form (rule:retroactive_3).

At every visit the provider asks whether anything has changed (rule:each_visit). Coverage works only in California and ends on moving away (rule:pharmacy_ca_only).

Covered: birth control including long-acting reversible contraceptives, emergency contraception and sterilization; counseling and education; STI testing and treatment; HIV testing; cervical cancer screening; limited fertility services. Not covered: emergency room visits, hospital stays, pregnancy care, and care unrelated to family planning (rule:services).

Refused? The pages read do not state an appeal process or deadline (rule:appeal).
