County indigent health programs (CMSP, Connect to Care, Healthy San Francisco: county care for uninsured adults): who qualifies, what you get, how to apply, and what happens next

California counties must provide health care to poor residents who have no other source. In 35 mostly rural counties the County Medical Services Program (CMSP) covers uninsured adults 21 to 64 with income up to 300% of the poverty level, regardless of immigration status; San Francisco runs Healthy San Francisco for residents up to 500%. Apply at the county social services office or a participating clinic.

What County indigent health programs is, in one paragraph

California law makes every county responsible for the care of poor residents who have no other support (rule:county_duty_17000). Counties meet that duty in different ways. Thirty-five mostly rural counties pool it in the County Medical Services Program (CMSP), which covers uninsured low-income adults who are not eligible for other public programs, and run Connect to Care, a primary and preventive care program for adults just above Medi-Cal income (rule:cmsp_definition, rule:cmsp_counties, rule:connect_to_care). San Francisco runs Healthy San Francisco, which is not insurance but a medical home with low-cost care for uninsured residents (rule:hsf_not_insurance). Other counties run their own programs (rule:other_counties).

300%
of the federal poverty level: the CMSP income limit in its 35 counties, 2026 ($64,945.98 a year for a household of two) (CMSP, rule:cmsp_income)

Who may qualify

County first. CMSP serves these 35 counties: Alpine, Amador, Butte, Calaveras, Colusa, Del Norte, El Dorado, Glenn, Humboldt, Imperial, Inyo, Kings, Lake, Lassen, Madera, Marin, Mariposa, Mendocino, Modoc, Mono, Napa, Nevada, Plumas, San Benito, Shasta, Sierra, Siskiyou, Solano, Sonoma, Sutter, Tehama, Trinity, Tuolumne, Yolo and Yuba (rule:cmsp_counties).

CMSP (rule:cmsp_income, rule:cmsp_age, rule:cmsp_immigration): ages 21 to 64; income at or below 300% of the poverty level, which is $64,945.98 a year for a household of two in 2026; available regardless of immigration status. Connect to Care (rule:connect_to_care): ages 21 to 64, income above 138% and up to 300% (above $29,875.15 up to $64,945.98 for two people), not enrolled in Medi-Cal or private insurance.

Healthy San Francisco (rule:hsf_eligibility): living in San Francisco, 18 or older, uninsured, not eligible for Medi-Cal or Medicare, and household income at or below 500% of the poverty level. It charges a quarterly fee per person by income (rule:hsf_fees):

Household sizeFee $0 (up to 100%)Fee $60 (to 200%)Fee $150 (to 300%)Fee $300 (to 400%)Fee $450 (to 500%)
1$1,330$2,660$3,990$5,320$6,650
2$1,804$3,608$5,410$7,216$9,017
3$2,277$4,554$6,830$9,108$11,384
4$2,750$5,500$8,250$11,000$13,750
5$3,224$6,448$9,670$12,896$16,117
6$3,697$7,394$11,090$14,788$18,484
7$4,170$8,340$12,510$16,680$20,850
8$4,644$9,288$13,930$18,576$23,217

The figures are the top of each monthly income band, from the March 2026 poverty level. Point-of-service fees are added at visits for income above 100%.

How to apply, and what happens after

CMSP: apply at the county social services or human services department, or at a participating enrollment provider; the application can be started on BenefitsCal (rule:cmsp_apply). Bring proof of residency, income and resources (rule:cmsp_documents). The county checks Medi-Cal first. An approved person gets a Notice of Action with the start and end dates, a benefits card, a CMSP ID card and a member guide; all non-emergency care must be from CMSP network providers, and some services carry a share of cost (rule:cmsp_issuance, rule:cmsp_share_of_cost). One month of retroactive coverage can be requested (rule:cmsp_retro). To continue, reapply during the last month of the term (rule:cmsp_reapply). Connect to Care: apply at a participating community health center.

Healthy San Francisco: enroll with a Certified Application Assistor at an enrollment site, pay the quarterly fee, report income or household changes, and reapply every 12 months before the anniversary date (rule:hsf_renew_12, rule:hsf_report). Enrollment ends on getting insurance, moving away, or becoming eligible for Medi-Cal or Medicare (rule:hsf_disenroll).

Disagree with a decision? The decision comes on a county Notice of Action; the CMSP pages read do not state the appeal deadline (rule:cmsp_appeal).

The same facts open other programs

County, age, income and insurance status are the facts county care runs on. The same facts are asked first by every health program in California.
