Home-delivered groceries / medically tailored meals (CalAIM Community Support) (Medi-Cal plan meals and groceries for diet-sensitive health conditions): who qualifies, what you get, how to apply, and what happens next

Medi-Cal managed care plans in California can cover medically tailored meals, groceries and supportive food for members with chronic or serious diet-sensitive conditions such as diabetes, heart failure or kidney disease. The service covers up to two meals or packages a day for up to 12 weeks, renewable if medically necessary, when the member’s plan offers it in their county. You ask your Medi-Cal plan; a denial can be appealed to the plan within 60 days.

What medically tailored meals is, in one paragraph

Medically tailored meals are one of the CalAIM Community Supports that Medi-Cal managed care plans can offer. A member with a chronic or serious health condition that diet affects can get meals, groceries or food packages designed by a registered dietitian for that condition, delivered as part of their care. Each plan chooses which Community Supports it offers in each county, so the member’s plan and county decide whether the service is available. It is a health benefit for a condition, not a food-insecurity benefit; CalFresh and WIC are separate.

Up to 2 meals a day, 12 weeks
medically tailored meals or food packages per authorization, renewable if medically necessary (DHCS, rule:two_per_day, rule:twelve_weeks)

Who may qualify

Three things decide it: Medi-Cal, the condition, and the plan.

Medi-Cal: an eligible Medi-Cal member (rule:medi_cal_member).

Condition: a chronic or serious condition that diet affects, such as cancer, heart disease or heart failure, diabetes or gestational diabetes, kidney disease, asthma or COPD, HIV, hypertension, high cholesterol, liver disease, malnutrition, obesity, stroke, digestive disorders, high-risk pregnancy, or a chronic or disabling mental or behavioral health disorder (rule:conditions). Food insecurity alone does not qualify (rule:not_food_insecurity_alone).

Plan: the member’s managed care plan must offer it in the county; plans choose among 14 Community Supports (rule:plan_offers). DHCS publishes the elections by plan and county.

No income test beyond Medi-Cal itself is stated (rule:no_income_test).

How to apply, and what happens after

Ask the Medi-Cal managed care plan, or ask a doctor or care manager to refer you. A registered dietitian assesses the condition and needs (rule:rdn_assessment).
